Welcome!

This year, a student in your family participated in the Future Well Kids program. Future Well Kids and the Abbott Fund aim to help people live fuller, healthier lives. Together with your student's school, teachers and Abbott volunteers we worked to empower students and deliver curriculum that encourages physical fitness and proper nutrition.

We know many of a student's most important habits are practiced and expanded in their home environments. This survey is intended to learn more about your family's approach to health and wellness and to encourage dialogue following the Future Well Kids visit in school. We appreciate your willingness to provide feedback, and encourage you to involve your student in completing this survey if possible.
